When comparing adult hernia surgery options, distinguish laparoscopic and open surgery, which describe the surgical approach, from mesh, which is used to reinforce the weakened area. Open surgery can also use mesh. Comparing “laparoscopic surgery versus mesh” as if they were alternatives can be confusing, so it helps to consider the approach and reinforcement separately. This article focuses on adult inguinal hernias; your own procedure is decided after assessment.
How do laparoscopic and open approaches differ?
Laparoscopic surgery uses small incisions to insert a camera and surgical instruments. Open surgery reaches the hernia through an incision close to it. Both return the protruding tissue to its proper position and repair the weakened area. American College of Surgeons: adult groin hernia repair

| Point to compare | Laparoscopic surgery | Open surgery |
|---|---|---|
| Approach | A camera and instruments are inserted through small incisions. | An incision is made close to the hernia. |
| Anesthesia | Usually performed under general anesthesia. | Options other than general anesthesia may also be considered. |
| What to discuss | Whether it suits your previous surgery and the location and extent of the hernia. | Whether it suits your hernia and anesthesia plan. |
The table compares general principles of anesthesia and choosing an approach. It does not cover every hospital’s detailed techniques, so the recommendation for your case needs a separate discussion during your appointment. Guy’s and St Thomas’ NHS: inguinal hernia repair
Consider mesh separately from the surgical approach
Mesh is a treatment material used to reinforce the hernia site and is widely used in adult inguinal hernia repair. Open repairs can use mesh reinforcement or repair the area with sutures. This is why you still need to ask about mesh even if you are having open surgery. American College of Surgeons: repair methods and mesh

If mesh is recommended, ask why reinforcement is needed, what benefits are expected and what you should be aware of. For a non-mesh approach, ask which conditions make it suitable and why it would be chosen. When considering surgical outcomes, look at whether the approach suits your case as well as whether a particular material is used.
What factors guide the choice?
During a surgical consultation, we consider the hernia’s size and location, whether it affects one or both sides, previous hernia or abdominal surgery, your health and the anesthesia plan. A laparoscopic approach may be considered for hernias on both sides or a recurrence after a previous open repair. Even then, the decision takes all these factors into account rather than relying on one condition alone. Guy’s and St Thomas’ NHS: choosing a hernia repair approach
If you have had surgery before, tell us the approach and the site. Bring any available operation records and examination results, so we can review them together when discussing the options.
Ask about recovery and the possibility of recurrence
Alongside the expected incisions and scars, ask about activity restrictions and pain management during recovery. You also need to understand the possibility of recurrence and complications such as infection. When deciding on a procedure, discuss the risks that may apply to you and the care needed afterwards. Pain and recurrence remain possible with any approach. Guy’s and St Thomas’ NHS: risks of hernia repair
